Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years in behavioral health, social services, or human services.
• 2+ years in children's mental health or related public sector field.
• Willingness to travel within Northeast Ohio up to 50% of the time.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate referred member needs/eligibility and determine approach to care.
• Collaborate with Medical Directors and care teams to overcome barriers.
• Provide coaching and support to empower member decision-making.
• Serve as single point of contact for members and remediate gaps in care.
