CVS Health Corporation in Ohio is seeking a Case Management Coordinator for a full-time, field-based telework role. You will travel up to 50% within Northeast Ohio to member homes and locations, supporting children and families through care coordination and education.

Requires a bachelor’s degree or non-licensed master, 2+ years in behavioral health, strong MS Office skills, and ability to travel. Residency in specified counties is preferred; weekend/after-hours work may be needed.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related field.
• 2+ years in family systems, case management, or related areas.
• 2+ years with Ohio delivery systems and resources.
• Willing and able to travel within Northeast Ohio up to 50%.
• Reliable transportation.
• Willing to work beyond core hours when needed.

• Be clinically and culturally competent in managing complex cases in the community.
• Evaluate member needs and coordinate care plans.
• Enhance medical appropriateness and quality of care; collaborate with teams.
• Monitor, evaluate and document care; adhere to regulatory guidelines.
