Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time, field-based telework role in Ohio. You will travel within Northeast Ohio to member homes and locations up to 50% of the time, supporting children and families through comprehensive care coordination.

Requirements include a bachelor’s or non-licensed master’s degree and 2+ years in behavioral health or related fields, with strong MS Office skills and knowledge of Ohio delivery systems.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related public sector field.

Responsibilities

• Evaluate referred members’ needs and eligibility and recommend care plan approaches.
• Coordinate and monitor care plan progress and activities.
• Collaborate with Medical Directors and other programs to overcome barriers and improve outcomes.
• Serve as a single point of contact for members and assist in remediating gaps in care.
