Job summary

Intermountain Healthcare Inc. is seeking a Care Management Assistant in Salt Lake City to optimize patient care in collaboration with nurses and social work care managers. You will coordinate appointments, facilitate communications with physicians and families, and educate patients about available resources.

The role supports transition planning, regulatory/payer communications, and care planning while assisting with community referrals to address social determinants of health.

Qualifications

• Demonstrated healthcare experience in a clinic or hospital setting.
• Strong customer service and communications focus with problem resolution.
• Proficiency in advanced computer skills.
• BLS certification, valid driver’s license, auto insurance, an acceptable driving record and reliable transportation.
• Spanish language ability is preferred.

Responsibilities

• Acts as liaison between department and external organizations, payers, physicians, patients, and families.
• Transmits clinical information for service authorizations per contractual requirements.
• Monitors referrals and maintains ongoing communication with care managers, revenue cycle, and payers.
• Retrieves medical records for retrospective utilization reviews or quality assurance.
• Delivers regulatory notices to patients within required timeframes and documents accordingly.
• Supports care management by coordinating transfer planning and arranging follow-up with families.
