Job summary

You will oversee social work, transitions of care, and utilization management while ensuring regulatory compliance and workforce development. Travel may be needed for oversight and leadership support across markets.

Qualifications

• Current RN license or LCSW/LMSW or applicable state license with relevant experience.
• BSN or MSW from an accredited institution.
• Care Management Certification within 24 months of hire or promotion.
• Demonstrated care management experience across settings.

Responsibilities

• Lead care management operations across utilization review, acute and emergency care, and ambulatory settings.
• Collaborate with medical, nursing, and revenue cycle leaders to optimize care and financial performance.
• Drive process improvements and monitor KPIs for quality and safety.
• Ensure regulatory compliance and workforce development within the department.
• Oversee social work, transitions of care, and utilization management.
