Job summary

University of Utah Health Hospitals and Clinics is seeking a care coordinator to manage the complex patient journey, coordinating multi‑specialty appointments, imaging, and tests across the care continuum.

The role acts as a patient liaison, triaging needs and ensuring timely, integrated care in collaboration with physicians and clinical teams at the Sugarhouse Health Center in Salt Lake City.

Qualifications

• Associate’s degree or two years’ healthcare coordination experience.
• Minimum of 3 years of experience in healthcare coordination or an administrative role.

Responsibilities

• Coordinates patient care across providers, specialties and settings.
• Liaises between patients, physicians and clinical teams; triages calls.
• Manages surgery scheduling and OR times; coordinates supplies.
• Oversees referrals and prior authorizations for tests and procedures.
• Gathers medical records, images and labs for new/return patients.
• Maintains clinic reports, waitlists and patient-facing documentation.
• Educates patients on procedures, resources and financials via MyChart.
• Manages Epic work queues and e-faxes; coordinates outreach workflows.
• Scrubs schedules for procedural accuracy; confirms records.
• Supports patient needs with internal/external programs as needed.
