Job summary

Qualifications

• Master of Social Work (MSW) from an accredited institution (degree verification required).
• Current state licensure, CSW or LCSW, obtained prior to/completion of required supervision hours.
• Basic computer proficiency with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking skills.
• Caregivers conducting home or community visits must maintain BLS, a valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assessment & Screening: Evaluates patients for transition planning, mental health and goals of care.
• Care Coordination: Develops and monitors care plans addressing social determinants and resources.
• Behavioral Health Support: Uses motivational interviewing and therapeutic techniques.
• Therapeutic Intervention: Provides brief individual, group and family therapy and psychosocial assessments.
• Diagnosis & Referrals: Identifies disorders and connects patients to services.
• Education & Advocacy: Trains staff, educates patients, and facilitates safe transitions to community.
• Team Collaboration: Works with healthcare teams, insurers and community providers.
• Quality & Compliance: Leads improvement initiatives and ensures policy adherence.
• Advanced Care Planning: Facilitates goals of care discussions with patients and families.
