Responsibilities

• Act as liaison between department and external payers, physicians, and families.
• Coordinate appointments and transmit clinical information for service authorizations.
• Monitor referrals and communicate with care managers, payers, and teams.
• Deliver regulatory notices and assist with patient education and resource navigation.
• Prepare transfer packets, arrange transportation, and support transition planning.
• Support care management and social work in implementing care plans.
