Job summary

Intermountain Health in Utah is seeking a Social Work Care Manager I to perform psychosocial assessments, develop care plans, and provide crisis-intervention counseling in collaboration with the care team. The role requires MSW and valid licensure, with an emphasis on transitions of care and community resource coordination.

The position offers a $2,500 sign-on bonus for external applicants with 1+ year of experience, and a 36-hour work week.

Qualifications

• Master of Social Work (MSW) from an accredited institution.
• Current state licensure (CSW/LCSW) obtained prior to or upon completion of required supervision hours.
• Basic computer proficiency with word processing and spreadsheet software.
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking.
• Caregivers conducting home or community visits must maintain BLS, valid driver’s license, auto insurance, driving record and reliable transportation.

Responsibilities

• Assesses patients and screens for transition planning, mental health, substance use, and goals of care.
• Develops and monitors care plans addressing social determinants of health and community resources.
• Provides brief individual, group, and family therapy as indicated.
• Identifies mental health conditions and connects patients to services.
• Trains staff, educates patients, and advocates for rights and access to care.
• Collaborates with healthcare teams, insurers, and community providers for quality care.
• Leads quality improvement initiatives and ensures policy adherence.
• Facilitates goals-of-care discussions with patients, families, and teams.
