Molina Healthcare, Inc. is hiring a Behavioral Health Care Coordinator to support care management and coordination across the care continuum in the Miami area.

You will collaborate with a multidisciplinary team to guide member progress toward outcomes and ensure high-quality, cost-effective care. The role involves travel (25-40%), conducting assessments, developing care plans with the member and providers, and monitoring plan effectiveness.

Formación

• 2+ years health care experience, preferably in behavioral health.
• Active and unrestricted professional license or eligibility in the state.
• Experience with care coordination, discharge planning, and home/community resources.

Responsabilidades

• Conduct behavioral health assessments within regulatory timelines.
• Develop and implement care plans with the member and care team.
• Perform telephonic, in-person, or home visits as required.
• Monitor progress, document interventions, and adjust plans as needed.
• Maintain an ongoing member caseload and outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Lead interdisciplinary care team meetings and ICT collaboration.
• Use motivational interviewing to support behavior change.
• Identify barriers to care and provide coordination assistance.
• Provide consultation and resources to peers.
