Job summary

The role supports care management activities across the continuum and the member’s progress toward defined outcomes. The position requires conducting assessments, developing care plans, and performing home visits or telephonic contacts as needed, with travel up to 40% based on state requirements.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active state license or equivalent.
• Valid and unrestricted driver’s license and reliable transportation for job-related travel.
• Experience with persons with severe mental health concerns, SUD, and foster care.
• Knowledge of whole person care, chronic conditions, and discharge planning coordination.
• Data entry skills and experience using a clinical platform.
• Excellent verbal and written communication skills.
• Microsoft Office proficiency.

Responsibilities

• Completes comprehensive behavioral health assessments per regulated timelines and determines eligibility for care coordination.
• Develops and implements a care plan with member, caregivers, and healthcare professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions, and suggests changes.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and ICT collaboration.
• Uses motivational interviewing and Molina clinical guideposts to educate and encourage change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation and recommendations to peers.
• 25-40% estimated local travel may be required.
