Molina Healthcare, Inc. is seeking a Behavioral Health Care Manager to support care coordination across the continuum. You will work with the multidisciplinary team to ensure member progress toward outcomes and align services for integrated care delivery.

The role involves conducting assessments, developing care plans, and performing home visits as needed, with travel depending on state contracts. Strong clinical credentials and experience in behavioral health are required.

Qualifikationen

• At least 2 years health care experience, preferably in behavioral health, or equivalent combination of relevant education and experience.
• Licensed behavioral health clinician credentials must be active and unrestricted in state of practice.
• Valid and unrestricted driver’s license and adequate auto insurance for job-related travel.
• Experience with severe mental health concerns and substance use disorder.
• Knowledge of whole person care, chronic conditions, and discharge planning coordination.
• Data entry and experience with a clinical platform.

Aufgaben

• Completes comprehensive behavioral health assessments and determines care coordination eligibility.
• Develops and implements care plans with member and care team.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Maintains ongoing member caseload for outreach and management.
• Promotes integration of behavioral health with LTSS and community resources.
• Facilitates multidisciplinary care team meetings and ICT collaboration.
• Educates and motivates members using motivational interviewing and clinical guidelines.
• Assesses barriers to care and provides coordination support.
• May provide consultation and resources to peers.
• 25-40% travel may be required.
