Job summary

Qualifications

• Minimum 2 years health care experience, preferably behavioral health.
• Licensed behavioral health clinician with active license in state of practice.
• Valid driver’s license and reliable vehicle for travel.
• Experience with severe mental health concerns and SUD.
• Proficient in Microsoft Office.
• Data entry experience with clinical platforms.

Responsibilities

• Complete comprehensive behavioral health assessments within regulated timelines.
• Develop and implement care plans with member and care team.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plan effectiveness and document outcomes.
• Maintain a regular member caseload and outreach.
• Promote integration of behavioral health with LTSS and community resources.
• Lead interdisciplinary care team meetings and ICT collaboration.
• Use motivational interviewing to support change.
• Assess barriers to care and provide coordination assistance.
• Provide consultation and resources to peers as needed.
• Travel 25–40% as required by state contracts.
