Molina Healthcare, Inc. is seeking a behavioral health care manager to support care coordination and collaboration across a multidisciplinary team.

You will help ensure member progress toward outcomes and contribute to strategies for quality and cost-effective care delivery. Key duties include conducting comprehensive assessments, developing care plans with members and caregivers, visiting members as needed, and monitoring plan effectiveness.

Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Active professional license in state of practice as specified.
• Valid driver's license and reliable transportation.
• Experience with severe mental health conditions and SUD.
• Knowledge of whole person care and discharge planning.

Responsibilities

• Completes behavioral health assessments within regulated timelines.
• Develops and implements care plans with member and team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness and documents interventions.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health and LTSS.
• Facilitates interdisciplinary care team meetings.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and coordinates solutions.
• May provide consultation to peers as needed.
• Travel 25-40% may be required.
