Job summary

Qualifications

• Licensed behavioral health clinician with active license in state of practice.
• Experience with care coordination or case management.
• Proficient in data entry and using clinical platforms.

Responsibilities

• Completes behavioral health assessments per timelines and identifies care coordination candidates.
• Develops and implements care plans with member, caregivers and professionals.
• Conducts telephonic, in-person or home visits as required.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Manages an ongoing member caseload with regular outreach.
• Promotes integration of behavioral health, LTSS and home/community resources.
• Facilitates care team meetings and inter-disciplinary collaboration.
• Uses motivational interviewing and Molina guidelines to support change.
• Assesses barriers to care and provides coordination assistance.
• May provide consultation to peers as needed.
• 25-40% local travel may be required.
