Job summary

Moda Health seeks a RHTP Primary Care Implementation Specialist to bridge Moda Health and rural hospital-based primary care clinics on Alaska’s Kenai Peninsula. You will drive PCMH recognition and APM design, enabling robust data reporting, population health management, and sustainable VBP participation.

The role requires collaboration with hospital stakeholders, project management, and technical assistance to meet NCQA PCMH criteria within a value-based care framework.

Qualifications

• Bachelor’s or Master’s degree in Public Health, Public Policy, or Healthcare Administration-related field preferred, or equivalent combination of education and relevant work experience.
• 3 - 5 years of experience in healthcare, public health, or health-related community-based organizations. Experience in rural/underserved communities preferred.
• Familiarity with NCQA PCMH and/or Oregon’s Patient-Centered Primary Care Home requirements and recognition processes.
• Understanding of value-based payment (VBP) systems and APMs in healthcare settings.
• Knowledge of primary care clinic functions, EHR systems, population health metrics and reporting, behavioral health integration, and THWs.
• Proficiency in Microsoft Office, especially MS Excel and other reporting software.
• Strong verbal, written and interpersonal communication skills.
• Experience convening diverse stakeholders and facilitating meetings (virtual/in-person).
• Ability to manage complex projects, track activities, and meet milestones.

Responsibilities

• Serve as subject matter expert to partner hospital leadership on PCMH recognition and APM design/implementation.
• Regularly meet with steering committee of Alaska hospitals and Moda Health experts to guide PCMH implementation.
• Use project management to create and maintain detailed implementation plan and trackers.
• Provide technical assistance to partners to achieve PCMH criteria and APM design.
• Collaborate with Medicaid Quality/Grants Management to support partners in meeting RHTP requirements.
• Use independent judgment to advise on PCMH criteria implementation and APM design.
