Job summary

Moda Health seeks a Grants Management & Implementation Specialist to support its Alaska Rural Health Transformation Program. The role coordinates grant activities, ensures compliance with federal/state rules, and builds partner capabilities to meet NCQA PCMH standards and VBP goals.

The position requires a master’s or equivalent experience, with strong project management and stakeholder facilitation. Hybrid schedule with travel to Alaska may be needed; based in Portland area or Alaska.

Qualifications

• Bachelor’s degree required; Master’s preferred or equivalent.
• 3–5 years in healthcare/public health or related community orgs.
• 2–3 years in grant management incl. monitoring and reporting.
• Familiarity with NCQA PCMH or Oregon PCMH recognition.
• Understanding of value-based purchasing (VBP) and alternative payment models.
• Knowledge of primary care clinic functions and EHR usage.
• Strong Excel and reporting skills.
• Excellent verbal and written communication.
• Ability to manage multiple complex projects.

Responsibilities

• Grants Management & Compliance: align activities with requirements.
• Grant Reporting & Evaluation: collect, validate, analyze data; report outcomes.
• Program Implementation & Technical Assistance: provide TA and develop training.
• Partner Engagement: coordinate meetings with agencies and partners.
• Other duties as assigned; growth and professional development.
