Job summary

Moda Health seeks a RHTP Primary Care Implementation Specialist to bridge Moda Health with rural hospital clinics on Alaska’s Kenai Peninsula. You will guide PCMH readiness, manage data reporting, and advance value-based payment alignment with NCQA PCMH models.

The role targets Alaska-based candidates and involves cross-organizational collaboration, project oversight, and performance improvement activities to prepare clinics for sustainable VBP participation.

Qualifications

• Bachelor’s degree or Master’s degree in Public Health, Public Policy, or Healthcare Administration-related field preferred, or an equivalent combination of education and relevant work experience.
• 3 - 5 years of experience working in healthcare, public health, or health-related community-based organizations. Experience working in or with rural or underserved communities preferred.
• Familiarity with NCQA PCMH and/or Oregon’s Patient-Centered Primary Care Home requirements and recognition processes.
• Demonstrated understanding of value-based payment (VBP) systems and APMs in healthcare settings.
• Knowledge of primary care clinic functions such as case management/care coordination, use of electronic health record (EHR) systems, population health-level metrics and reporting, behavioral health integration, and collaboration with Traditional Health Workers (THWs).
• Proficiency in Microsoft Office, especially MS Excel and other reporting software.
• Strong verbal, written and interpersonal communication skills.
• Experience convening diverse stakeholders and facilitating productive virtual and in-person meetings.
• Ability to manage complex projects, track activities, and meet short and long-term project milestones.
• Ability to maintain confidentiality and project a professional business image.

Responsibilities

• Serve as subject matter expert to partner hospital organization leadership on all matters related to PCMH recognition and Alternative Payment Model (APM) design and implementation.
• Regularly meet with steering committee composed of executive leadership from both Alaska hospitals, Moda Health subject matter experts, and statewide policy partners to guide PCMH implementation, resolve barriers, and ensure shared ownership of key decisions.
• Use project management skills to create and maintain detailed implementation plan and task trackers across multiple stakeholder organizations.
• Provide technical assistance to partner organizations, enabling achievement of core and elective criteria for PCMH recognition across the concepts of team-based care and practice organization, knowing and managing patients, patient-centered access, care management and support, care coordination and transitions, performance measurement and quality improvement, and behavioral health integration.
• Partner closely with Medicaid Quality and Innovation Manager, Grants Management Specialist, and Medicaid Quality Supervisor to support clinical partners in meeting RHTP requirements and timeline.
• Use independent judgment to make key business decisions and provide informed recommendations on PCMH criteria implementation and APM design to partner organizations and Moda Health senior leadership.
• Perform other duties as assigned.

Position Summary

The RHTP Primary Care Implementation Specialist serves as a key liaison between Moda Health and partners at rural hospital-based primary care clinics on Alaska’s Kenai Peninsula. This role provides technical assistance, subject matter expertise, and project management oversight to advance value-based payment (VBP) readiness through the National Committee on Quality Assurance (NCQA) Primary Care Medical Home (PCMH) implementation. The position partners with hospital and clinic stakeholders to implement a robust quality improvement and data reporting infrastructure to enable quality measure reporting, support population health management, and prepare clinics for sustainable VBP participation through the NCQA PCMH model.

This is a full-time position. The ideal candidate will reside in Alaska’s Anchorage/Kenai Peninsula region. This is a limited duration position contingent on continued receipt of RHTP grant funds.

Pay Range

$85,000 - $87,318.58 annually (depending on experience). Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Working Conditions & Contact With Others

• Office environment with extensive close PC and keyboard use, constant sitting, and frequent phone communication. Must be able to navigate multiple computer screens. A reliable, high-speed, hard-wired internet connection required to support remote or hybrid work. Must be comfortable being on camera for virtual training and meetings. Work in excess of standard workweek, including evenings and occasional weekends, to meet business need. Occasional and overnight travel may be required.
• Internally with all Moda departments. Externally with partner healthcare organizations and vendors.
