Job summary

Moda Health seeks a skilled RHTP Primary Care Implementation Specialist to liaison between Moda Health and Alaska rural clinics on Kenai Peninsula. You will support VBP readiness, PCMH criteria, data reporting, and population health management.

Qualifications

• Bachelor’s or Master’s degree in Public Health, Public Policy, or Healthcare Administration-related field preferred, or an equivalent combination of education and relevant work experience.
• 3 - 5 years of experience in healthcare or health-related organizations; rural/underserved experience preferred.
• Familiarity with NCQA PCMH recognition processes.
• Understanding of VBP systems and APMs in healthcare.
• Knowledge of primary care clinic operations, EHRs, population health metrics, and care coordination.

Responsibilities

• Serve as subject matter expert to partner hospital leadership on PCMH recognition and APM design.
• Meet with steering committee to guide PCMH implementation and shared decisions.
• Create and maintain detailed implementation plans and task trackers.
• Provide technical assistance enabling core and elective PCMH criteria.
• Collaborate with Medicaid Quality and Grants Management to meet requirements.
• Offer informed recommendations on PCMH criteria and APM design to leadership.
• Perform other duties as assigned.
