Job summary

Moda Health is seeking an RHTP Primary Care Implementation Specialist to act as a liaison between Moda Health and rural hospital-based primary care clinics on Alaska’s Kenai Peninsula. The role focuses on advancing VBP readiness via NCQA PCMH implementation, building data reporting infrastructure for quality measures and population health management.

The ideal candidate will have 3–5 years in healthcare, strong project management, and experience with EHR and reporting tools.

Qualifications

• Bachelor’s or Master’s in Public Health, Public Policy, or Healthcare Administration preferred.
• 3–5 years in healthcare, public health, or health-related community-based orgs; rural experience preferred.
• Familiarity with NCQA PCMH and/or Oregon PCMH requirements and recognition processes.
• Demonstrated understanding of value-based payment (VBP) systems and APMs in healthcare.
• Knowledge of primary care clinic functions: case management, EHR, population health metrics, THWs collaboration.
• Proficiency in Microsoft Office, especially Excel, and reporting software.
• Strong verbal, written and interpersonal communication skills.
• Experience convening diverse stakeholders and facilitating meetings (virtual/in-person).
• Ability to manage complex projects, track activities, and meet milestones.

Responsibilities

• Serve as liaison between Moda Health and rural hospital clinics to advance VBP readiness.
• Provide technical assistance, subject matter expertise, and project management oversight for NCQA PCMH implementation.
• Develop and maintain quality improvement and data reporting infrastructure to enable reporting and population health management.
• Prepare clinics for sustainable NCQA PCMH participation and related reporting requirements.
