Job summary

Moda Health is seeking an RHTP Primary Care Implementation Specialist to act as a liaison between Moda Health and rural hospital-based primary care clinics on Alaska’s Kenai Peninsula. The role focuses on advancing VBP readiness via NCQA PCMH implementation, building data reporting infrastructure for quality measures and population health management.

The ideal candidate will have 3–5 years in healthcare, strong project management, and experience with EHR and reporting tools.

Qualifications

• Bachelor’s or Master’s in Public Health, Public Policy, or Healthcare Administration preferred.
• 3–5 years in healthcare, public health, or health-related community-based orgs; rural experience preferred.
• Familiarity with NCQA PCMH and/or Oregon PCMH requirements and recognition processes.
• Demonstrated understanding of value-based payment (VBP) systems and APMs in healthcare.
• Knowledge of primary care clinic functions: case management, EHR, population health metrics, THWs collaboration.
• Proficiency in Microsoft Office, especially Excel, and reporting software.
• Strong verbal, written and interpersonal communication skills.
• Experience convening diverse stakeholders and facilitating meetings (virtual/in-person).
• Ability to manage complex projects, track activities, and meet milestones.

Responsibilities

• Serve as liaison between Moda Health and rural hospital clinics to advance VBP readiness.

Job description

The RHTP Primary Care Implementation Specialist serves as a key liaison between Moda Health and partners at rural hospital-based primary care clinics on Alaska’s Kenai Peninsula. This role provides technical assistance, subject matter expertise, and project management oversight to advance value-based payment (VBP) readiness through the National Committee on Quality Assurance (NCQA) Primary Care Medical Home (PCMH) implementation. The position partners with hospital and clinic stakeholders to implement a robust quality improvement and data reporting infrastructure to enable quality measure reporting, support population health management, and prepare clinics for sustainable VBP participation through the NCQA PCMH model.

This is a full-time position. The ideal candidate will reside in Alaska’s Anchorage/Kenai Peninsula region. This is a limited duration position contingent on continued receipt of RHTP grant funds.

Required Skills, Experience & Education:

• Bachelor’s degree or Master’s degree in Public Health, Public Policy, or Healthcare Administration-related field preferred, or an equivalent combination of education and relevant work experience.
• 3 – 5 years of experience working in healthcare, public health, or health-related community-based organizations. Experience working in or with rural or underserved communities preferred.
• Familiarity with NCQA PCMH and/or Oregon’s Patient-Centered Primary Care Home requirements and recognition processes.
• Demonstrated understanding of value-based payment (VBP) systems and APMs in healthcare settings.
• Knowledge of primary care clinic functions such as case management/care coordination, use of electronic health record (EHR) systems, population health-level metrics and reporting, behavioral health integration, and collaboration with Traditional Health Workers (THWs).
• Proficiency in Microsoft Office, especially MS Excel and other reporting software.
• Strong verbal, written and interpersonal communication skills.
• Experience convening diverse stakeholders and facilitating productive virtual and in-person meetings.
• Ability to manage complex projects, track activities, and meet short and long-term project milestones.
• Ability to maintain confidentiality and project a professional business image.
