Job summary

Central Peninsula Hospital, Inc. is seeking a Program Manager, PCMH, to coordinate and sustain our new Patient-Centered Medical Home program across multiple primary care clinics in Soldotna and the Kenai Peninsula.

You will lead centralized care management operations, supervise staff, and collaborate with clinical leadership, IT, and partners to improve access and outcomes. This grant-funded role focuses on program development, staff education, workflow implementation, performance monitoring,

Qualifications

• Associate’s degree in Healthcare Administration, Business Administration, Project Management, Public Health, or a related field.
• 2 years of experience in a clinical or hospital setting.
• 1 year of experience in a progressively responsible role in a healthcare setting.
• Valid State of Alaska driver’s license.

Responsibilities

• Coordinate, implement, and sustain the PCMH program across participating primary care clinics.
• Oversee centralized care management operations and supervise assigned program staff.
• Collaborate with clinical leadership, IT, quality, and external partners to support patient-centered care.
• Monitor performance, drive quality improvement, and report on objectives.
