AltaMed Health Services Corporation in Pico Rivera, CA is seeking a Personal Care Attendant II to assist with daily living activities across care settings, including center and home visits. The role may require flexible scheduling and supporting care coordination with the PACE program.

The ideal candidate will have a Certified Nursing Assistant education, state certification, and bilingual abilities in English and Spanish (Mandarin or Cantonese depending on location).

Formación

• Certified Nursing Aide education and training is required.

Responsabilidades

• The PCA II will perform and assist with personal care ADLs/IADLs for participants in the PACE across all platforms of care (center, home, or virtual).
• The PCA II supports the PCA III in gathering and reporting information related to care coordination and participant well-being.
