Job summary

AltaMed Health Services is seeking a Personal Care Attendant (PCA) II to perform and assist with personal care ADLs/IADLs for participants in the PACE, across centers, homes, and virtual delivery. The PCA II will support the PCA I and collaborate with the care team to advance participant independence and wellbeing.

The role requires a CNA education/license, a high school diploma, and flexibility to work with diverse populations.

Qualifications

• Certified Nursing Aide education and training.
• High school diploma required or some college education/credits preferred.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location, preferred.
• A minimum requirement of a valid BLS certification or higher, following AHA or Red Cross guidelines.

Responsibilities

• Perform and assist with personal care ADLs/IADLs for participants in the PACE across all care delivery platforms, including center, home, or virtual.
• Support the PCA I duties as needed and assist the PCA III with care coordination and reporting.
• Collaborate with IDT members to align on the plan of care and promote participant independence, wellbeing, and satisfaction.
