Job summary

AltaMed Health Services Corporation invites applications for a Personal Care Attendant (PCA) II to assist with personal care and daily living activities for participants in the PACE program across center, home, and virtual care delivery models.

The PCA II supports the PCA III in care coordination and communicates with IDT members to meet the plan of care and enhance participants’ independence and well-being.

Qualifications

• Certified Nursing Aide education and training.
• Nurse Assistant Certificate by the State of California (DHHS-Aid and Technician Certification).
• High school diploma required or some college education preferred.
• California Food Handler Certificate preferred (must be obtained within 30 days).
• Experience with elderly/physically or mentally impaired preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred depending on location.

Responsibilities

• Perform and assist with personal care ADLs/IADLs for participants in PACE across care delivery platforms.
• Support PCA I duties as needed and assist with care coordination information gathering and reporting.
• Collaborate with IDT members to align with the plan of care, focusing on needs and well-being.
