Job summary

AltaMed Health Services Corp. is seeking a Personal Care Attendant (PCA) II to perform and assist with personal care ADLs/IADLs for participants in the PACE, across all care delivery platforms including center, home, or virtual visits.

The PCA II will support the PCA III in care coordination, gathering and reporting information, and communicating with IDT members to address participant needs and goals, fostering independence and engagement.

Qualifications

• Certified Nursing Aide education and training.
• High school diploma required or some college education/credits preferred.
• California Food Handler Certificate often required within 30 days of employment.
• Prior experience working with elderly or impaired populations preferred.
• Bilingual abilities may be preferred depending on location.

Responsibilities

• Perform and assist with personal care ADLs/IADLs for participants in the PACE.
• Support care coordination and reporting within the plan of care.
• Communicate and collaborate with IDT members regarding participant needs and status.
