Job summary

AltaMed Health Services Corp. seeks a Personal Care Attendant II to perform and assist with ADLs/IADLs for participants across centers, homes, or virtual care. The PCA II may cover PCA I duties when needed, and supports care coordination with the team to improve participant engagement and independence.

The role requires California CNA education and a valid BLS certification, with bilingual proficiency depending on location, and a strong background in elder care among the candidates.

Qualifications

• Certified Nursing Aide education and training required.
• Nurse Assistant Certificate by the State of California required.
• BLS certification required or higher per AHA/Red Cross guidelines.

Responsibilities

• Perform personal care (ADLs/IADLs) for participants across all delivery platforms.
• Support care coordination and reporting to the care team.
• Communicate with IDT members to align on plan of care and participant needs.
