Job summary

AltaMed Health Services Corp. is seeking a Personal Care Attendant (PCA) II to perform and assist with personal care ADLs/IADLs for participants in the PACE program across all care delivery platforms, including center, home, or virtual.

The PCA II may cover PCA I duties and supports care coordination with IDT members. Qualified candidates will have a CNA certification, appropriate California certification, and a high school diploma or equivalent.

Qualifications

• Certified Nursing Aide education and training.
• Nurse Assistant Certificate by the State of California, DHHS- Aid and Technician Certification.
• High school diploma required or some college education/credits preferred.
• California Food Handler Certificate of Achievement within 30 days of employment.
• Prior experience with elderly or disabled individuals preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred depending on location.

Responsibilities

• Perform and assist with personal care ADLs/IADLs for participants in PACE.
• Support PCA I duties when assigned and assist care coordination goals.
• Communicate with IDT members regarding participant needs and status.
