Job summary

HCA Healthcare in Florida is seeking a Patient Transfer Logistics Specialist to coordinate intake of referrals, provide timely updates, and support patient admissions and transfers across Emergency Departments, inpatient settings, or community settings.

You will respond to calls, faxes, and emails, gather initial information, track bed availability, and facilitate transfers with a sense of urgency and strong customer service.

Qualifications

• 2+ years of healthcare experience highly preferred
• Behavioral Health experience highly preferred
• Call Center - Transfer Center - Bed Management experience helpful

Responsibilities

• Responds to phone calls with urgency and customer service
• Gathers information to open new cases and prepares clinical packets
• Locates resources and coordinates information to support patient disposition and admissions
• Maintains bed availability inventory within the community for non-HCA bed placements
• Accepts, tracks and facilitates hospital transfers and transports via phone and computer
• Provides updates to customers on status of patient cases
• Assists to schedule and prioritize discharge transports and supports rapid response to service requests
• Demonstrates critical thinking in patient acuity and flow through the continuum of care
