Qualifications

• High school diploma or GED required.
• Six months in a fast-paced office environment with multitasking ability.
• BLS certification from AHA to be obtained within 90 days of hire.
• Excellent customer service and telephone communication skills.
• Proficient using computer software; ability to protect confidential information.

Responsibilities

• Participates in daily huddles and supports patient care coordination.
• Serves as EHR super user and troubleshooter for clinic systems.
• Coordinates scheduling, referrals, and health records maintenance.
• Assigns patients to PCPs and maintains scheduling with providers.
• Ensures access to Health Information Exchange and retrieves pertinent data.
• Completes patient care documentation in the EHR in real time.
• Handles multiple priorities in a busy clinic and supports team initiatives.
• Performs other duties as assigned.
