CenterWell Home Health is seeking a Patient Services Coordinator to orchestrate patient scheduling, coordinate with field staff and clinicians, and support day-to-day office operations.

The role requires strong communication, organization, and multitasking skills, plus at least 1 year of home health experience; coding certification is a plus. You will manage schedules, handle infection control forms, and assist with case conferences to ensure continuity of care.

Qualifications

• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsabilités

• Manages schedules for all patients and edits for coverage, updating agent unavailability.
• Initiates infection control forms and routes reports to the appropriate teams.
• Back-up coverage during peak times and assists with field communications and weekly case conferences.
• Maintains the hospitalization log and coordinates notes across office, field, and sales staff.
• Schedules and coordinates various visits (discharge, add-ons, supervision) and ensures supervisory visits are scheduled.
