Job summary

Qualifications

• Must have excellent communication skills and the ability to interact with diverse groups.
• At least 1 year of home health experience.
• Packet review or QA/QI experience is preferred.
• Coding certification is preferred.

Responsibilities

• Schedules visits for all patients and edits schedules when staff are sick or unavailable.
• Communicates with field staff, patients, physicians to coordinate care and maintain continuity.
• Provides back-up support during lunch hours and busy periods, handling calls from field staff.
• Maintains the hospitalization log and shares coordination notes with the team.
• Completes scheduled tasks and ensures staff are assigned appropriate visits (nurse/injection).
• Generates and reviews scheduling reports and weekly agent schedules for productivity and geography.
