Job summary

Claremedica Health Partners, LLC is seeking a Referral Coordinator to schedule and manage referral appointments, ensuring smooth coordination with outside specialists and health plans. You will review referrals, obtain prior authorizations as needed, and assist patients with barriers to care.

Strong HIPAA adherence and customer service are essential. You will act as the patient advocate, maintain documentation, and help families navigate the healthcare system in a fast-paced office environment

Qualifications

• High school diploma or GED required.
• 1 year of experience processing referrals or front desk/clinical experience preferred.
• HIPAA compliance and confidentiality required.
• Excellent oral and written communication, time management and organization.
• Ability to communicate professionally with staff, patients and providers; bilingual is a plus.

Responsibilities

• Schedule referral appointments and coordinate with outside specialists.
• Maintain knowledge of health plan referral requirements and approvals.
• Provide excellent service to patients face-to-face or over the phone.
• Track referrals and document information for team awareness and safety.
• Serve as a system navigator and help patients overcome barriers to care.
• Assist with obtaining medical information, prior authorizations, and records as needed.
• Communicate with reviewers and carriers to ensure approvals.
