Davis Street Primary Care Clinic is seeking a patient navigator to bridge clinic services with community resources. You will coordinate care across primary care, social services, and community providers to guide patients to comprehensive, integrated, and person-centered care.

You will perform patient outreach, coordinate appointments and referrals, collaborate with program leaders, and support high-risk patients needing enhanced navigation.

Qualificações

• Experience in healthcare, case management or social services.
• Knowledge of community resources and integrated care models.
• Strong communication, telephone etiquette and cultural sensitivity when helping patients.

Responsabilidades

• Coordinate care across clinic services, social services and community providers.
• Assist with patient outreach and engagement for Davis Street programs.
• Schedule and manage referrals among primary care and community partners.
• Maintain HIPAA-compliant documentation and protect patient information.
• Support clinic reception duties and general administrative tasks as needed.
