Job summary

Davis Street Primary Care Clinic is seeking a patient navigator to bridge clinic services with community resources. You will coordinate care across primary care, social services, and community providers to guide patients to comprehensive, integrated, and person-centered care.

You will perform patient outreach, coordinate appointments and referrals, collaborate with program leaders, and support high-risk patients needing enhanced navigation.

Qualifications

• Experience in healthcare, case management or social services.
• Knowledge of community resources and integrated care models.
• Strong communication, telephone etiquette and cultural sensitivity when helping patients.

Join our Davis Street Primary Care Clinic team and help bridge healthcare and community resources for patients in our community.The patient navigator coordinates care across clinic services, social services, and community-based providers to guide patients to receive comprehensive, integrated, and person-centered care. This role helps patients access services and overcome barriers to care.Conduct patient outreach and engagement on Davis Street programs, services and other supportive community resources.Coordinate appointments and referrals among primary care and other Davis Street and community providers.Collaborate with Davis Street clinic and program leaders to support patient needs with Davis Street staff and healthcare providers.Support high-risk patients requiring enhanced patient navigation.Advocate for patient needs and access to services.Build partnerships with Davis Street program leaders, community agencies and service providers.Maintain accurate and timely documentation and track activities and quality outcomes ensuring compliance with HIPAA and confidentiality standards.May perform clinic reception duties and administrative support as neededAssist with special projects and clinic initiatives as requested.Other duties as assignedAssociates or Bachelor’s degree preferred.Experience in healthcare, case management, or social services.Knowledge of community resources, healthcare systems, and integrated care models.Skillful with many facets of communication and interaction with patients including; telephone techniques, etiquette, tact, professionalism and cultural sensitivity while providing a positive and supportive experience.Adheres to privacy, confidentiality, and security policies and procedures related to patient Protected Health Information (PHI).Commitment to the mission and values of Davis Street.
