Qualifications

• Associate degree or higher preferred
• Experience in an acute care setting preferred
• Understanding of patient flow through hospital settings
• Strong verbal and written communication
• Good organizational skills; multitasking ability

Responsibilities

• Respond to phone calls with urgency and strong customer service
• Gather initial information to open cases and prepare clinical packets
• Maintain knowledge of facility staff, services and programs
• Locate resources and coordinate information for patient disposition
• Maintain bed availability inventory for non-HCA placements
• Facilitate hospital transfers and transports via phone and computer
• Provide status updates on patient cases and bed placements
• Schedule and prioritize discharge transports for the Patient Logistics Center
