Job summary

Qualifications

• High School Diploma or GED required.
• One year experience in an acute care setting required.
• Two years experience in a health care setting preferred.

Responsibilities

• Facilitate placement of patients as directed by supervisors and physicians.
• Coordinate transfers to appropriate facility and unit.
• Identify barriers to timely placement and escalate to leadership.
• Communicate with referring and receiving facilities to ensure timely placement of patients.
