Elite Home Health is seeking an Intake Coordinator to manage the intake and referral process for new patients entering home health services in Florida. You will verify information, ensure documentation is complete, and collaborate with clinical and administrative teams to facilitate timely admissions in compliance with Medicare and state regulations.

The role is administrative and coordination-focused, with no direct patient care.

Qualifications

• Two years of healthcare intake/admissions or home health operations experience.
• Experience with EMR systems such as Axxess, WellSky, or Kinnser preferred.
• Strong organizational and communication skills; ability to pass background screening and compliance requirements.

Responsabilités

• Process new patient referrals from hospitals, physicians, facilities, and community partners.
• Coordinate scheduling of initial patient visits with clinical staff and ensure required documentation is obtained prior to admission.
• Verify patient insurance coverage and eligibility; assist with authorizations as required by payers and communicate coverage requirements to teams.
• Ensure intake documentation is complete, accurate, and HIPAA-compliant; maintain audit readiness.
