Job summary

Qualifications

• High school diploma or equivalent required; healthcare-related education preferred.
• Experience in healthcare intake, referral coordination, admissions, or insurance verification preferred.
• Knowledge of home health/private duty workflows; familiarity with insurance verification processes and medical terminology.

Responsibilities

• Process incoming referrals via telephone, fax, hospital portals, secure email, and electronic referral systems.
• Field incoming private duty lead calls and route leads to care coordination.
• Review referral documentation for completeness and accuracy; obtain required clinical and demographic information.
• Communicate referral status, next steps, and service availability to patients, families, referral sources, and internal teams.
• Verify insurance eligibility and benefits for Home Health and Private Duty services.
• Document benefit details, coverage limitations, and authorization requirements.
• Collaborate with clinical leadership, scheduling, operations, and sales to facilitate transitions from referral to start of care.
• Coordinate with clinicians to address referral appropriateness, timing, and service needs.
• Maintain confidentiality of PHI per HIPAA and company policies.
