Job summary

Green Meadows Home Health is seeking an Intake Coordinator to manage referrals from initial contact through admission, coordinating patient information, verifying eligibility, and ensuring documentation is complete.

The role requires collaboration with clinical staff to facilitate timely admissions while complying with federal, state, and agency regulations. Strong communication and organizational skills are essential.

Qualifications

• High school diploma or equivalent required; higher degree preferred.
• 1+ year in healthcare intake/admissions or related customer service.
• Experience with EMR and knowledge of Medicare/Medicaid guidelines.
• Excellent verbal and written communication, strong organizational skills.

Responsibilities

• Receive and process referrals from physicians, hospitals, and other sources.
• Review documentation for completeness and accuracy.
• Verify patient demographics, insurance coverage, and eligibility.
• Coordinate with physicians for orders, certifications, and docs.
• Schedule admissions and communicate services to patients and families.
• Enter data accurately into the EMR system and maintain records.
• Communicate admission status with referral sources and staff.
• Ensure compliance with Medicare/Medicaid/HIPAA policies.
• Assist with authorizations and insurance verification as needed.
• Coordinate clinician assignment with scheduling staff.
• Provide exceptional customer service to patients, families, and partners.
• Participate in quality improvement and departmental meetings.
