Job summary

Grady Health System in Atlanta seeks a Clinical Care Coordinator (C3) to proactively identify and remove barriers to department or system-wide patient flow, serving as a consultant and educator to medical staff on patient status, bed placement, and payor requirements.

The HUB C3 oversees system-wide coordination 24/7, partnering with nursing units and service departments to manage admissions, transfers, and discharge processes, ensuring timely, patient-centered placement and efficient LOS.

Responsibilities

• Evaluates bed requests and assigns bed and physician treatment team by priority to facilitate patient flow.
• Efficiently places each patient and verifies he/she is in the appropriate level of care and correct patient status/type.
• Coordinates with the Unit C3, Surgical C3 and ED C3 to facilitate patient placement including all admissions, transfers and discharges on a 24/7 basis.
• Provides centralized one-call service for direct admits and facility transfers.
• Determines Diagnosis Related Group (DRG) initial Length of Stay (LOS) values for direct admissions and facility transfers (all status and types)
• Verifies admission orders are complete and includes a clear admission status order in a timely manner.
• Proactively anticipates, identifies, and addresses barriers that may impede system-wide flow, and escalates issues to appropriate individuals/departments.
• Assists Order Organizer (O2) with priorities regarding patient flow to ancillary testing and procedure areas.
• Manages process for direct admissions and transfers who do not meet admission criteria.
