Job summary

Grady Memorial Hospital Corporation is seeking a Clinical Care Coordinator (C3) to optimize patient flow and serve as a liaison across departments and facilities. The role focuses on bed placement, patient status, LOS, and swift coordination of admissions, transfers, and discharges.

As part of the HUB C3, you will work with medical staff and multiple service locations to ensure efficient, patient-centered outcomes and improved LOS performance for the organization.

Responsibilities

  • Evaluates bed requests and assigns bed and physician treatment team by priority to facilitate patient flow.
  • Efficiently places each patient and verifies he/she is in the appropriate level of care and correct patient status/type.
  • Coordinates with the Unit C3, Surgical C3 and ED C3 to facilitate patient placement including all admissions, transfers and discharges on a 24/7 basis.
  • Provides centralized one-call service for direct admits and facility transfers.
  • Determines Diagnosis Related Group (DRG) initial Length of Stay (LOS) values for direct admissions and facility transfers (all status and types)
  • Verifies admission orders are complete and includes a clear admission status order in a timely manner.
  • Proactively anticipates, identifies, and addresses barriers that may impede system-wide flow, and escalates issues to appropriate individuals/departments.
  • Assists Order Organizer (O2) with priorities regarding patient flow to ancillary testing and procedure areas.
  • Manages process for direct admissions and transfers who do not meet admission criteria.

Job Summary

The Clinical Care Coordinator (C3) proactively identifies and removes barriers that may impede department or system-wide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The HUB C3has a system wide view of the hospital, including 24/7 central coordination of patient admission, placement, flow, staffing and service department testing and procedures. The nursing units and service departments are the spokes that work with the HUB to manage patient progression at a local level. The HUB C3 will act as the primary liaison between all service locations, support departments, other healthcare facilities and in partnership with physicians to ensure appropriate patient placement for effective, efficient patient-focused outcomes. Works collaboratively with all members of the healthcare team to improve length of stay (LOS) performance of the organization.
