Grady Health System in Atlanta, GA is seeking a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and serve as a liaison among emergency, surgical services, nursing, and care teams.
The role focuses on coordinating patient progression, validating care levels, and communicating the plan of care to patients and families, with RN background and nurse case management experience preferred.

The Clinical Care Coordinator (C3) proactively identifies and removes barriers that my impede department or systemwide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The position (Unit C3) will act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, utilization review to ensure appropriate bed is assigned, patient care needs are met and treatment plans executed at the appropriate levels. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.
