Job summary

Grady Health System is seeking a Clinical Care Manager with a hospital-wide perspective to oversee 24/7 central coordination of patient admissions, discharges, throughput, and staffing.

The role partners with nursing, service locations, and physicians to ensure optimal patient placement and quality outcomes. You will manage budget, payroll, patient satisfaction, and employee engagement while driving throughput, LOS, and readmission reduction across the organization.

Qualifications

• Bachelor's Degree in a health related field.
• Master's Degree in a health related field preferred.
• Six (6) or more year’s job related experience including minimum three (3) years of Case management experience. One (1) year of supervisory experience
• Registered Nurse in State of Georgia
• Basic Life Support (BLS)
• Case Management Certification Preferred

Responsibilities

• Coordinat​ion and Facilitation: Manage all aspects of Care Coordination plans including patient progression, barrier escalation and resolution, continuous process improvement initiatives, operations and training.
• Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met, and treatment plans are executed in the appropriate level of the continuum.
• Leadership: Provide leadership, direction, resources, training and education, evaluation and support to staff. Responsible for staff selection, retention, and development.
• Collaboration and Optimization: Collaborate with Nursing, Social Services, Environmental Services, Transportation, Physicians, and other disciplines to facilitate patient placement including admission, transfers and discharges.
• Assist Care Coordination leadership in identifying, monitoring, and evaluating department operations to ensure optimal efficiency and effectiveness.
• Compliance: Partner with Quality Leadership to conduct quality assurance reviews of patient charts to verify compliance with all governing regulations and quality metrics.
