Job summary

Grady Health System seeks a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and serve as a consultant with medical staff on patient status, bed placement, payer requirements and resource use. The HUB C3 maintains a system-wide view and coordinates admissions, transfers, and discharges around the clock.

In this role you will partner with physicians and service locations to optimize LOS and patient-focused outcomes, supporting unit C3 and ED C3 teams while promoting

Responsibilities

• Evaluates bed requests and assigns bed and physician treatment team by priority to facilitate patient flow.
• Efficiently places each patient and verifies appropriate level of care and patient status.
• Coordinates with Unit C3, Surgical C3 and ED C3 for 24/7 admissions, transfers and discharges.
• Provides centralized one-call service for direct admits and facility transfers.
• Identifies and escalates barriers to system-wide flow and works with departments to resolve them.
