Job summary

Grady Health System in Atlanta, GA is seeking a Clinical Care Coordinator (C3) to proactively identify barriers to patient flow and serve as a liaison among emergency, surgical services, nursing, and care teams.

The role focuses on coordinating patient progression, validating care levels, and communicating the plan of care to patients and families, with RN background and nurse case management experience preferred.

Qualifications

• Bachelor's degree required.
• 2+ years of RN experience.
• Nurse Case Management experience preferred.

Responsibilities

• Acts as liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, utilization review.
• Identifies and removes barriers to bed placement and patient flow.
• Reviews patient status and coordinates progression plans with physicians and nursing staff.
• Communicates with patients and families regarding plan of care and discharge goals.
• Collaborates with MSW and CDI teams to progress patient care toward discharge.
