Responsibilities

• Coordinate patient flow activity and safe transfers of care across the health and care system.
• Maintain accurate patient tracking systems and data quality.
• Monitor pressures and risks, escalating concerns as needed.
• Support admission avoidance and discharge planning processes.
• Produce reports and presentations to inform decision making.
• Organise and support multidisciplinary team meetings; ensure actions/outcomes are followed up.
• Collaborate with community, acute, social care and voluntary sector partners.
