Job summary

Rochester Regional Health is seeking a Discharge & Patient Flow Coordinator to facilitate patient throughput and discharge planning across care settings. This role collaborates with the care team to implement the discharge plan and coordinates with social workers, nurses, providers, and insurers to ensure seamless transitions.

The position focuses on reducing bed days, managing housing logistics, and leveraging the Care Connect EMR to align patients with appropriate levels of care.

Qualifications

• Minimum 1 year experience in health care marketing with knowledge of skilled nursing facility admissions and/or hospital case management.
• Bachelor's Degree required.
• Ability to coordinate with social workers, nurses, providers, insurers, ElderOne and facilities to ensure continuity of care.
• Experience with discharge planning and bed management helpful.
• Strong communication and ability to maintain confidentiality.

Responsibilities

• Manage discharges and create open bed resources with bed availability and census status.
• Coordinate with social workers, nurses, providers, insurers, ElderOne, RRH SNFs, and external agencies to ensure continuity of care.
• Link residents being discharged with follow-up healthcare services.
• Assist with housing and homelessness mitigation by planning proactively.
• Build and maintain relationships vital to project success.
• Use the EMR Care Connect system to coordinate with RRH facilities and ElderOne for appropriate care levels.
