Rochester Regional Health seeks a Discharge & Patient Flow Coordinator to streamline patient throughput and discharge planning. You will collaborate with clinical teams to implement discharge plans and ensure continuity of care for patients and their families.

The role focuses on reducing unnecessary bed days, coordinating with social workers, insurers, and external providers, and using the Care Connect EMR to align care levels with patient needs.

Qualifications

• Minimum 1 year experience in health care marketing and knowledge of skilled nursing facility admissions and/or hospital case management
• Bachelor’s Degree required
• For ElderONE: 1 year experience with the frail and elderly preferred
• For ElderONE: Bachelor’s Degree preferred.

Responsabilités

• Manage discharges and create open bed resources; coordinate with admins and clinicians on bed status and escalations.
• Coordinate with social workers, nurses, providers, insurers, ElderOne, SNFs and external agencies to ensure continuity of care.
• Link residents being discharged with follow-up healthcare resources.
• Assist with establishing housing and mitigate homelessness proactively.
• Build and grow business relationships vital to project success.
• Use EMR Care Connect to coordinate with facilities and ElderOne for appropriate care levels.
• Demonstrate knowledge of insurance coverage (Medicare, Medicaid, commercial).
• Identify and address discharge barriers through communication and troubleshooting.
• Maintain data on databases and documentation.
• Develop best practices and tools for project execution and management.
