Columbia Memorial Hospital is seeking a Transitions of Care and Patient Flow Coordinator to facilitate efficient patient movement and timely discharges. You will partner with Case Management, Nursing, Physicians/Providers, and support services to resolve barriers to flow and coordinate care across the interdisciplinary team.

Qualifications

• Bachelor's Degree in Nursing required
• Current NYS Registered Nurse License; Certification in Case Management preferred
• Experience in an acute hospital setting with patient flow, discharge planning, care coordination, and hospital operations

Responsibilities

• Serve as a liaison to facilitate efficient patient movement throughout the hospital.
• Collaborate with Case Management, Nursing, Physicians/Providers, Ancillary Services and others to identify and resolve barriers to patient flow and discharges.
• Promote timely progression of care and align teams by communicating priorities and escaping barriers.
